Job summary

Intercept Health is seeking a Life Skills Coach/Case Manager for the Young Adult Independent Living Program in Roanoke, VA. You will engage with residents aged 17–21, model positive values, and support transitions into the community.

You will help with money management, employment readiness, education goals, daily living skills, and career development while contributing to the resident living plan and handling transportation and administrative duties.

Qualifications

• The position requires a Bachelor's degree in any field plus two years of case management experience with children and families.
• Or a bachelor's degree in human services with relevant case management experience, or a human services Master's with student placement in related settings.

Responsibilities

• Engage with residents aged 17–21 to model and teach social skills and values.
• Support residents in money management, employment, and educational goals.
• Assist residents with daily living skills and career development opportunities.
• Contribute to the development and execution of the resident living plan.
• Provide transportation support and handle related administrative duties.

As a Full-time Life Skills Coach at YouthQuest's Independent Living Apartment Program, you will actively engage with young adults, ages 17-21, who are preparing for successful transition into the community. You will serve as a positive role model for residents to observe and learn socially effective values, attitudes, and behaviors. You will support residents in developing money management skills, obtaining employment, completing their education as applicable, learn daily living skills, and explore career development opportunities. You will contribute to the formulation and implementation of the resident transitional living plan and assist with life skills learning, transportation, and administrative duties.

Education and Experience Required:

The selected candidate will have a Bachelor's diploma in any field plus two years of experience in providing case management services to children and families; OR a human service Bachelor's degree (Social Work, Psychology, Counseling, Special Education, etc) with experience providing case management services to children/youth and their families preferred; OR a human service Master's degree (Social Work, Psychology, Counseling, Special Education, etc) with a student placement providing case management services to children/youth and their families.
