Job summary

Center for Human Services (CHS) is seeking a dedicated case manager to divert and assist youth ages 18-24 at the Youth Navigation Center in Modesto, CA. You will coordinate services, perform assessments, and support housing stability through individualized plans.

Responsibilities include intensive case management, linking with emergency and permanent housing, and skill-building for independent living and employment. Strong communication and data reporting are essential to success.

Qualifications

• Experience working with youth ages 18-24 in case management or supportive coaching.
• Strong written and oral communication skills.
• Computer literacy with Word, Excel, and databases.

Responsibilities

• Assess new clients and process health plan referrals.
• Develop youth-driven plans to address barriers to housing stability.
• Provide intensive case management including counseling, advocacy, transportation.
• Assist clients with independent living and employment skills.
• Maintain client records and document progress daily.
• Track data and generate reports for program monitoring.
