Job summary
Center for Human Development (CHD) is seeking a Housing Support Specialist to join our DYS Transition to Independent Living Program (TILP) in Western Massachusetts. The role supports youth ages 17-22 aging out of foster care, including on-call coverage during evenings and weekends, and involves placement and ongoing casework in scattered-site apartments.
The position includes a $500 sign-on bonus and a competitive hourly rate.
Qualifications
- Experience working with youth/young adults preferred.
- Must have a vehicle and valid driver’s license; reliable transportation required.
- Bilingual English/Spanish candidates preferred.
Responsibilities
- Interview and screen young adult referrals.
- Complete intake processes and assessments; develop treatment plans and reviews.
- Maintain organized documentation with timelines and progress notes.
- Develop rapport with youth and set growth-oriented goals.
- Assist with housing placement, move-in/out activities, and apartment readiness.
- Coordinate with funding sources and community resources for youth needs.
- Provide trauma-informed counseling, positive youth development, and on-call coverage.
