Job summary

Center for Human Development in West Springfield, MA is seeking a Housing Support Specialist to join our DYS Transition to Independent Living Program (TILP) serving youth ages 17-22 aging out of foster care.

The role involves placement and casework across scattered-site apartments, on-call evenings/weekends, and coordinating with youth, landlords, and providers to promote safety, stability and independence.

Qualifications

• Associates or Bachelor's degree preferred.
• Passion for working with youth/teens.
• Vehicle and driver's license required.

Responsibilities

• Interview and screen referrals.
• Complete intake, assessments, and treatment plans.
• Maintain organized documentation with timelines.
• Develop rapport and goals for youth growth.
• Assist with moving in/out and securing rentals.
• Provide direct instruction of independent living curricula.
• Transport clients to appointments and meetings.
