Chicanos Por La Causa, Inc. (CPLC) in TX seeks a LPN/LVN/Medical Coordinator to provide direct nursing care and coordinate medical services for youth in our care.

You will monitor health needs, administer medications, and collaborate with healthcare providers to ensure continuity of care while maintaining regulatory compliance. You will perform nursing assessments, respond to medical concerns and emergencies, and educate youth on health and prevention.

Qualifikationen

• Current LPN/LVN license in good standing or BSN with accredited program.
• Minimum 1 year experience in hospital, clinic, or health care setting.
• Must have valid driver’s license and MVR/insurance eligibility.
• CPR and First Aid or BLS certification preferred.

• Provide direct nursing care and coordinate medical services for youth.
• Assess, facilitate, monitor medical needs, and manage medication administration.
• Educate youth on health status, prevention, and treatment plans.
• Schedule and track medical, dental, and specialty appointments.
