The LCDP will assess needs, deliver crisis support, and work with an integrated team to achieve treatment goals. Responsibilities include in-home counseling, progress monitoring, data entry, and coordinating with families, schools, and referring agents.

Qualifications

• Bilingual abilities are preferred (English, Spanish, Portuguese, Creole).
• Experience in human services and non-profit settings is valued.
• Demonstrates a safe, strengths-based approach while supporting youth and families.

Responsibilities

• Assess Seven Challenges case management needs for clients and families.
• Conduct bio-psychosocial assessments per agency policy.
• Collaborate with an integrated team to develop and review treatment goals.
• Provide in-home and community-based counseling addressing referrals and progress.
• Maintain crisis and trauma counseling and ensure client safety.
• Monitor and track client drug screenings and data entry in ECHO or reporting platforms.
• Uphold NASW boundaries and ensure cultural sensitivity.
• Coordinate reunification efforts and maintain professionalism with partners and courts.
