Job summary

Pacific Clinics in California is seeking a youth and family counselor to provide direct mental health services in home and community settings. You will work within a team to address maladaptive behaviors, linking families to resources and promoting strengths.

Responsibilities include documenting in MediCal-aligned systems, collaborating with parents and multi-disciplinary teams, and developing safety and skill-building plans. Must have a High School Diploma or GED and 1+ year of experience.

Qualifications

• High School Diploma or GED with 1+ year of relevant experience.
• Experience providing direct behavioral support to individuals with mental health or behavioral challenges.
• Familiarity with trauma-informed care and data collection for behavioral outcomes.
• Ability to work independently with multidisciplinary teams.
• Certification in CPI or PCM or willingness to obtain.

Responsibilities

• Provides direct services to youth and families.
• Documents in alignment with MediCal regulations and achieves set productivity expectations.
• Builds alignment with parents, youth, and others in the normal course of working with the families.
• Utilizes strengths of youth, families, and others to assist in the implementation and achievement of goals.
• Works with youth and family teams to support family driven, strength based planning and interventions.
