Job summary

Centerstone in Bradenton, Florida, seeks a youth services liaison to mentor and advocate for youth and their caregivers, coordinating linkage, monitoring, evaluation and advocacy for families involved in juvenile cases and mental health challenges. You will collaborate with therapists and community partners to strengthen family systems.

The role involves in-home, community, and office work with weekly contact, thorough documentation, and timely referrals.

Qualifications

• Education Level High School Diploma or GED required.
• Associate degree in Social work, Psychology or related human services field preferred.
• Years of Experience One year's experience working with at risk youth and/or caregivers of at risk youth.
• Child welfare experience a plus.
• Florida Driver License in good standing required.

Responsibilities

• Reports to work as scheduled on a daily basis to facilitate the development of therapeutic relationships with clients and to facilitate consistent client services.
• Maintains productivity requirement of 70% minimum.
• Maintains standards for direct client contact and interaction.
• Maintains client confidentiality according to established policies, federal and state statutes.
• Provides documentation for services rendered within established time frames including progress notes, assessments and treatment plans.
• Completes assigned paperwork in compliance with standards, including time sheets, mileage logs, leave requests, client records and MIS.
• Provides quality services to clients and families as evidenced by documentation.
• Meets with individuals assigned in home, community, and office setting at least weekly.
• Prepares client documentation on behalf of therapists, and documents same in client record.
• Makes and follows up on referrals to outside service providers on behalf of therapists, and documents same in client record.
