Qualifications

• High School Diploma or equivalent.
• Two years of youth/community experience.

Responsibilities

• Develop individualized plans to build social, life, and independent living skills.
• Collaborate with families, social workers, and community providers.
• Document progress and participate in case planning and supervision.
