Qualifications

• Bachelor’s degree in a related field such as counseling, social work, or a closely related discipline.
• At least two years’ experience in a behavioral health setting providing case management/community support services.
• Proficient use of electronic medical records, computers, telehealth systems, and cell phones.

Responsibilities

• Completes community needs and clinical assessments for all individuals on caseload.
• Participates in interdisciplinary treatment team meetings to inform client treatment needs.
• Provides ongoing case management and skill-building to help clients achieve goals.
• Maintains timely, high-quality documentation for service contacts.
• Outreaches to all clients assigned to caseload at least twice monthly.
• Facilitates communication between community providers and clients.
