Job summary

The Yunion is seeking a Care Manager in Detroit, MI for the C.A.R.E. Early Intervention Program. This contract role focuses on weekly engagement, care planning, advocacy, and coordination with families, schools, and partners to address barriers early on.

Responsibilities include relationship-based case management, intake/assessment, treatment planning, and documentation to support youth stability and positive outcomes.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Education, Human Services, Counseling, or related field.
• Minimum one year experience working with youth and families.
• Strong written and verbal communication skills.
• Valid driver’s license and reliable transportation required.

Responsibilities

• Manage a caseload of up to 20 youth per contract.
• Conduct regular face-to-face contact with youth and families.
• Develop and coordinate individualized treatment plans.
• Advocate for youth and families across systems.
• Maintain accurate case notes and progress reports.
• Coordinate referrals and collaboration with schools and community partners.
