Job summary

Fred Finch Youth & Family Services is seeking a CARE Manager to coordinate care, educate clients, and improve access to health and behavioral health services for children, youth, and families. The role involves advocacy, collaboration with interdisciplinary teams, and navigation of community resources in a culturally responsive manner.

Qualifications

• Minimum three years of relevant work experience.
• High school diploma or GED is required.
• Ability to work flexible hours including some night/weekend shifts.
• Capable of home visits and community outreach.
• Experience with EMR/EHRs and care management software.

Responsibilities

• Oversee specific cases and coordinate health care benefits.
• Provide education and facilitate access to care in a timely, cost-effective manner.
• Collaborate with clients, family supports, and other providers to promote wellness.
• Advocate for clients and liaison with community programs and providers.
