Job summary

LifePath Systems seeks a YCOT Specialist to provide crisis intervention and coordination for youth and families within the catchment. The role emphasizes trauma-informed care, rapid assessments, and linkage to services in collaboration with partners and supervisors.

Qualifications include a Master’s in counseling or social work, and licensure or eligibility (LPC/LCSW/LMFT) with English/Spanish bilingual preferred. Travel 20%–100% is required; evenings and varied shifts are possible.

Qualifications

• Master's degree in counseling, social work, or related field.
• Licensed/eligible for LPC, LCSW, or LMFT.
• English/Spanish bilingual preferred.
• Reliable transportation and valid driver's license.
• Experience with psychosocial education, skills training, or assessments.
• Ability to complete detailed progress notes and maintain documentation.

Responsibilities

• Conduct psychosocial and diagnostic assessments.
• Identify behavioral health needs and coordinate access to treatment.
• Provide trauma-informed crisis interventions and de-escalation.
• Collaborate with MCOT, CIS, and CIT to stabilize in least restrictive environments.
• Coordinate referrals and safety planning for youth in crisis.
• Maintain ongoing communication with families and service providers.
• Coordinate with ED, DFPS, and community resources to divert from inpatient care.
• Ensure seamless connection to community mental health resources.
