Job summary

LifePath Systems is seeking an LPHA Youth Crisis Outreach Team Specialist to provide crisis intervention and ongoing support for youth and families in crisis. You will conduct assessments, coordinate treatment options, and collaborate with MCOT/CIS/CIT teams to stabilize individuals in the least restrictive environment.

Remote travel and variable shifts are required; bilingual skills are a plus. The role emphasizes trauma-informed care, detailed documentation, and safety planning within Lifepath

Qualifications

• Master’s degree in counseling, social work, or related field required.
• Licensed Professional Counselor (LPC) / LCSW / LMFT preferred but not required.
• English/Spanish bilingual preferred; reliable transportation and valid driver’s license.
• Ability to write clear progress notes and maintain documentation standards.

Responsibilities

• Conduct psychosocial assessments and crisis interventions.
• Identify behavioral health needs and coordinate access to treatment.
• Develop and maintain contacts with youth, families, and partner providers.
• Provide trauma-informed interventions and crisis de-escalation strategies.
• Collaborate with multidisciplinary teams for stabilization and referrals.
