Job summary

New Beginnings is seeking a Community Case Manager in Lewiston, ME to provide case management for youth in the Maine Transitional Living Program (TLP). The role involves engaging with homeless youth, assessments, locating and maintaining housing, group facilitation, and collaboration with youth to secure positive outcomes.

The position requires MHRT/C certification and a minimum of one year with homeless or high-risk youth, with travel to Augusta and Farmington as needed.

Qualifications

• Bachelor's degree in a human-services related field or a bachelor's degree in a non-human service-related field with 2 years of Human services experience.
• Combined five years of education and experience in providing direct services in social, health or behavioral health fields.
• Current Mental Health Rehabilitation Technician/Community (MHRT/C) Certification.

Responsibilities

• Provide case management for youth in the Maine Transitional Living Program (TLP).
• Engage with homeless youth and develop individual service plans.
• Maintain a caseload up to 14 youths and document progress in case records.
• Collaborate with community partners and interns; assist with housing search and stability.
