Job summary

CITY MISSION is seeking a Case Manager to support residents in a Transitional Living Program. You will provide guidance, coaching, and skills development to youth, while coordinating program activities and maintaining a therapeutic environment.

The role requires three years in case management, CPR/First Aid certification, and knowledge of OCYF 3800 regulations. Bachelor’s in Human Services is preferred; weekend shifts may be required.

Qualifications

• Three years of experience in case management or related field.

Responsibilities

• Maintain a therapeutic environment within the Transitional Living Program.
• Coordinate daily program activities and staff communication.
• Review facility logs and logs related to operations; supervise program activities.
• Prepare weekly reports and manage applicable paperwork and records.
• Assist with transitions, safety, and well‑being of residents.
