Job summary

Responsibilities include intake processing, documenting case notes, coordinating visits, arranging transitions/discharges, providing transportation, and delivering aftercare up to 120 days, with on-call duties rotating.

Qualifications

• Bachelor's degree in social work or related field.
• One year of direct services or case management experience.
• DCFS/CANTS clearance required.

Responsibilities

• Participate in the intake process and ensure forms are completed and filed.
• Participate in pre-placement interviews and provide information to the treatment team.
• Document case notes in a timely manner per Medicaid timelines.
• Evaluate client progress and communicate with Youth Care Workers.
• Develop and modify individual treatment plans during clinical staffings.
• Coordinate client visits with families and approved resources.
• Plan transitions/discharges and update comprehensive transition plans.
• Provide transportation for interviews and appointments as needed.
• Deliver aftercare services for up to 120 days post-discharge.
• Attend required trainings and participate in quality improvement processes.
• Take on-call responsibility on a rotating basis.
