Job summary

BCFS Health and Human Services seeks a Transition Specialist who uses professional judgement to ensure quality case management for youth aging out of foster care, helping them set and reach achievable goals. You will assess needs, obtain resources, advocate with human services, health, and court systems, and develop Individual Service Plans.

The role requires confidentiality and strong communication skills, with occasional after-hours work as needed.

Qualifications

• Bachelor’s degree in Health and Human Services, Social Services, Education or Criminal Justice.
• Minimum 2 years of experience in case management, job development or crises intervention with youth.
• Experience in community building, event coordination and public speaking.

Responsibilities

• Assess the personal needs of PAL clients and help to obtain resources to meet those needs by providing case management services.
• Advocate with human services, health, and court and criminal justice systems to resolve issues facing clients.
• Assist youth in crisis by providing de-escalation and developing a plan with youth to move out of state crises.
• Develop detailed Individual Service Plans for each client and meet with them regularly to assess progress.
• Maintain timely and accurate file documentation and data entry.
• Follow all HHS guidelines and policies concerning PAL TFSS services.
• Demonstrate knowledge of contracts and agency policies.
