Job summary

Oasis Center in Nashville, TN seeks a Case Manager to support young adults ages 18-24 in rapidly moving from homelessness to stable housing through Housing First approaches.

You will manage a caseload, coordinate housing navigation, document interactions in HMIS/Salesforce, and collaborate with partners to secure housing and provide life skills training.

This non-exempt role offers benefits and the chance to help youth achieve self-sufficiency in a trauma-informed, anti-oppressive environment.

Qualifications

• Bachelor’s degree and one year of relevant experience.
• Experience with Housing First and Rapid Re-Housing preferred.
• Ability to form strong relationships with LGBTQ+ youth.
• Strong communicator and problem solver, with the ability to work with youth, staff and partners.
• Understanding of anti-oppression principles and trauma-informed care.

Responsibilities

• Manage a caseload of approximately 15–20 young adults and coordinate housing navigation and supportive services.
• Deliver services using a trauma-informed and positive youth development approach.
• Assist in preparing young adults for housing, including documentation and life skills training.
• Maintain HMIS and Salesforce records in a timely and ethical manner.
