Qualifications

  • Bachelor’s Degree in Human Services or Social Work or 5 years of experience in Human Services.
  • Up to date with all required trainings in current position.
  • A minimum of one year of childcare experience required.
  • Valid state issued driver's license.
  • Must be at least 21 years of age.
  • Legally authorize to work in the United States and provide verification.
  • Able to pass an Enhanced Background Check through the Idaho Dept. of Health & Welfare.
  • Within two months of hire date, completion of in-service training per agency policy

Responsibilities

  • Serve as a member of the Program Treatment Team with the goal of integrating clinical, milieu, and direct-care efforts.
  • Foster a Family-Centered approach working with family members of a resident.
  • Obtain and compile information from various sources and maintain a comprehensive record on each resident.
  • Advocate for the resident during placement ensuring health, safety and resident rights.
  • Coordinate care by liaising between multidisciplinary team members and residents.
  • Schedule and conduct multidisciplinary meetings and submit reports within set timeframes.
  • Maintain a current, comprehensive case record and update changes in status with relevant parties.
  • Provide direct service to residents including counseling, advocacy, discipline, and information.
