Job summary

The Idaho Youth Ranch is seeking a dedicated Case Manager to support youth and families in our Residential Center for Healing and Resilience. You will collaborate with clinical and medical teams to assess, plan, coordinate, and monitor services aligned with individualized treatment plans.

You will work in Idaho, providing case management for youth receiving inpatient residential care, ensuring confidentiality, HIPAA compliance, and linkage to community resources.

Qualifications

• Social Work Licensure or Bachelor's degree in behavioral science or related field with 1 year experience with children in a social service setting or 5 years experience with children in a social service setting.
• Ability to complete annual certification of CANS.
• Demonstrate knowledge of Medicaid Youth Empowerment Services

Responsibilities

• Provide case management for youth receiving inpatient residential care, assess, plan, link, coordinate, and monitor options and services that address a youth and family's needs.
