Job summary

Bellewood & Brooklawn is seeking a Case Manager to provide in-home and out-of-home case management across multiple programs, including Foster Care and Independent Living. Qualifications

• One year full-time employment experience working directly with individuals with behavioral health needs; master's degree may substitute for experience.

Responsibilities

• Provides in-home and out-of-home case management to a caseload set by supervisor.
• Coordinates, delivers, and documents services with outside providers and agency staff.
• Builds rapport with youth and families and conducts weekly contacts as required by program guidelines.
• Collaborates with youth, families, therapists, schools, and team members to assess needs and safety.
• Completes program assessments and develops plans of service together with youth and families.
