Job summary

Tri-Starr Talent is seeking a Youth Case Manager to support children in conservatorship by coordinating services and managing case plans within the San Antonio area.

You will assess safety, work with families and partners, and guide permanency outcomes such as reunification or adoption while maintaining detailed case notes and participating in trainings and meetings.

Qualifications

• Associate’s degree in Child Welfare or 60 college credit hours, and two years of relevant experience.
• Relevant experience may substitute for education.
• Ability to coordinate services with families and partners.

Responsibilities

• Manage a caseload of children and support permanency planning.
• Assess child safety, family needs, and placement options.
• Develop and implement service and permanency plans.
• Conduct home visits, family assessments, and meetings.
• Maintain accurate case notes and documentation.
• Coordinate services with internal teams, courts, and agencies.
• Provide guidance and ongoing support to children and families.
• Respond to crisis situations and provide on-call support.
• Participate in team meetings and required trainings.

Job description

The Youth Case Manager supports children in conservatorship by coordinating services, managing case plans, and working toward permanency outcomes such as reunification, kinship placement, or adoption. This position works closely with families, caregivers, and community partners to help ensure children are placed in safe, stable, and supportive environments.

Work Location: Field-Based Position – San Antonio and surrounding areas

Key Responsibilities

• Manage a caseload of children in care and support permanency planning efforts.
• Assess child safety, family needs, and placement options.
• Develop and implement service and permanency plans supporting reunification, kinship care, or adoption.
• Conduct home visits, family assessments, and meetings with families and service partners.
• Maintain accurate case notes, reports, and documentation in accordance with program requirements.
• Coordinate services with internal teams, courts, and community agencies.
• Provide guidance and ongoing support to children, families, and caregivers throughout the case process.
• Respond to crisis situations and provide on-call support as needed.
• Participate in team meetings, required trainings, and program activities.
