Job summary

Family Service of Rhode Island (FSRI) in Exeter, RI is seeking a Care Coordinator to manage health care appointments for residential students. You will schedule, prepare, staff and document medical, dental, vision and psychiatric visits, from admission through discharge, while maintaining clear family and guardian communications.

The role requires a bachelor's degree in social work or related field (or equivalent residential/childcare experience), strong organizational skills, and knowledge of

Qualifications

• Bachelor’s degree or relevant experience in social services is required.
• Experience coordinating medical care, scheduling, or case management preferred.
• Knowledge of Rhode Island Medicaid, EPSDT, and DCYF health processes is preferred.
• Flexibility for evenings/weekends as scheduled.
• Bi/Multilingual skills preferred; HIPAA confidentiality expected.
• Background checks required for employment in licensed residential programs.

Responsibilities

• Coordinate health care for all students: medical, dental, vision, psychiatric and specialty appointments.
• Staff, track and document appointments from admission through discharge.
• Maintain Building Bridges Initiative communications with families, DCYF and guardians.
• Advocate for students in navigating human service agencies and providers.
