Job summary

ROWI Teen & Parent Wellness Centers in Visalia, CA seeks a Care Coordinator to coordinate care for all ROWI clients under the supervision of the Program Director. You will arrange orientation, transportation, doctor visits, and scheduling to ensure seamless services.

The role emphasizes high standards of care, client supervision, facilitating groups, completing assessments, and timely EMR documentation. A valid driver’s license and experience with youth in therapeutic settings are required.

Qualifications

• High school diploma required; Bachelor's degree preferred.
• Minimum two years driving experience.
• Minimum 1 year working with kids in a therapeutic setting.
• Experience with junior high or high school students preferred.
• Ability to communicate with diverse backgrounds and handle stress.

Responsibilities

• Coordinate care for ROWI clients and arrange orientation, transportation, and doctor visits.
• Ensure services meet highest standards and clients attend designated groups.
• Facilitate assigned groups and activities as designated by the Facility Manager.
• Conduct client orientation and PHQ9 assessments as directed.
• Document shift reports and EMR entries, including transportation.
