Optum is seeking a Behavioral Health Advocate in Brooklyn to support Under-21 Medicaid members with case management and care coordination. You will meet clients at hospitals, clinics, community sites, or in homes, with 80% fieldwork and 20% remote work.

The role requires a Masters/Doctoral degree in a relevant field, NY licensure, and at least 2 years of mental health experience, including work with youth. Normal weekday schedule and residency in Brooklyn required.

Competenze

• Master's or Doctoral degree in Psychology, Social Work, Counseling, Marriage and Family Therapy, or a related field.
• Active, unrestricted NY license as LMHC, LMFT, LCSW or LP.
• Minimum 2 years of experience in a mental health setting.
• At least 2 years of experience working with the Under 21 population.
• Proficient in Microsoft Office (Word, Outlook, Excel, Teams).
• Dedicated home office setup with reliable high‑speed internet.
• Valid transportation for daily travel across an assigned territory.

Mansioni

• Identify and provide community service linkage.
• Assist enrollees with aftercare appointments.
• Provide follow-up as needed for individual members.
• Identify gaps in care and help connect to needed services.
• Review Census to identify re-admissions within 12 months.
• Attend facility meetings with treatment teams to plan aftercare.
• Manage aftercare follow-up for high‑risk enrollees.
• Meet with community providers to foster relationships and new programs.
