Job summary

Wyandot Behavioral Health Network in Kansas City, KS is seeking a Case Manager to provide goal‑directed services for young adults (18‑26) with serious mental illness, aiming to increase functional skills, reduce hospitalizations, and maximize independent community living. You will support clients in group and drop‑in activities and collaborate with EPIC team.

Qualifications

• Bachelor’s degree in social work, psychology, rehabilitation or related field.
• Experience in mental health preferred, especially with young adults.
• Strong interpersonal and written communication skills.

Responsibilities

• Engage individuals and families as partners and develop trusting relationships.
• Conduct assessments and participate in treatment plan development.
• Mediate between clients and other professionals to facilitate care.
• Travel to meet clients in the community and provide direct support.
• Complete documentation and reporting in a timely manner.
