Job summary

Center for Life Management (CLM) seeks a Young Adult Community Case Manager to provide care planning, resource coordination, and community-based counseling for clients aged 15-26 with mental health needs.

The role emphasizes collaboration with a multidisciplinary team, crisis intervention when needed, and thorough documentation. A relevant degree and direct service experience are preferred, with strong communication and organizational skills essential.

Qualifications

• Bachelor’s degree preferred with relevant field
• Associate’s degree with minimum two years direct service in a related field may qualify
• Ability to exercise sound clinical judgment in routine, urgent, and emergent clinical situations
• Proficient computer skills, specifically with all Microsoft Office applications
• Ability to document effectively and efficiently in clinical work
• Ability to navigate an electronic health record system
• Ability to work as part of a clinical team while also being self-directed

Responsibilities

• Provide care planning and service monitoring for clients aged 15-26
• Assist clients with applying for and maintaining medical coverage
• Provide outreach, advocacy, coordination and resource identification
• Deliver home, school, or community-based counseling support
• Provide Therapeutic Behavioral Services to develop coping strategies
• Engage families with training and counseling for resiliency and recovery
• Conduct crisis intervention when needed and ensure safety
• Document services promptly and accurately
• Collaborate with treatment teams and external providers
• Maintain professionalism and confidentiality per agency and state standards
