Job summary

Clifford Beers Community Care Center is seeking a Fatherhood Engagement Specialist to provide intensive outreach and wrap-around services to fathers referred by the Department of Children and Families. You will conduct assessments, develop care plans, and coordinate with family supports to promote positive parenting and engagement.

The role emphasizes trauma-informed, anti-racist practice, documentation in care-management systems, and collaboration across community partners in Connecticut.

Qualifications

• Minimum of 5 years of experience leading and directing the work of others.
• Knowledge of social services, neighborhoods, political landscape and community stakeholders throughout greater New Haven.
• Experience with individuals and families needing specialized services including health and social determinants of health; crisis and safety planning.
• Demonstrates family-centered approach and trauma-informed, anti-racist practices.
• Proficiency in documenting in care management technology platforms.
• Flexibility to adapt to changing program needs.

Responsibilities

• Provide wrap-around and trauma-informed service coordination.
• Facilitate the Parent Education Program, 24/7 Dad (twelve-week curriculum).
• Coordinate case conferences with DCF Social Worker and maintain Wraparound certification.
• Develop and manage family-centered Care Plan and Care Team Meetings.
• Maintain HIPAA-compliant data management and reporting; travel as needed within CT.
