The Wraparound Facilitator partners with youth, families, natural supports, providers, schools, and community agencies to identify strengths and needs, facilitate Child and Family Team meetings, develop and monitor individualized Wraparound Plans of care, coordinate services, and support families in achieving their goals. The Wraparound Facilitator promotes collaboration, community integration, crisis prevention and planning, and the development of sustainable natural supports while ensuring services are delivered in accordance with YES Waiver, agency, contractual and state requirements.
