Home Base Inc is seeking a Wraparound Facilitator/Case Manager in Ravenswood, WV. Qualifications
Valid driver’s license and reliable transportation.
Ability to pass NCIC/CIB and APS/CPS background checks.
Three professional references.
2–4 years of case management and/or behavioral health experience preferred.
1–2 years of experience leading teams preferred.
Experience providing direct services to children and families preferred.
Responsibilities
Provide wraparound services to a caseload of up to 15 families.
Coordinate supports, resources, and services using a strengths-based approach.
Facilitate team meetings with youth, families, and multidisciplinary partners.
Conduct and administer the Child and Adolescent Needs and Strengths (CANS) assessment.
Maintain accurate, timely, and compliant documentation.
Skills
Strong written and verbal communication skills
Proficiency with Outlook, Word, Excel, and PDF platforms
Professionalism and strong customer service skills
Ability to work independently with minimal supervision
Experience with virtual platforms
Education
Bachelor’s Degree in a human services field
