Job summary

Spurwink is seeking a Wraparound BHH Coordinator in Lewiston, ME to partner with families, schools, community providers, and natural supports to help youth succeed at home and in their communities.

The role leads the High-Fidelity Wraparound process, coordinating a multidisciplinary team, developing individualized Plans of Care, and ensuring fidelity through regular meetings and documentation.

Qualifications

• Bachelor's degree in social work, psychology, counseling, or related field; or related degree with relevant experience.
• Must complete Wraparound training.
• Experience coordinating multidisciplinary teams is preferred.

Responsibilities

• Facilitate High Fidelity Wraparound process with family-driven, youth-guided approaches.
• Lead Child and Family Team meetings and coordinate participation of youth, families, and supports.
• Develop, implement, monitor, and update Wraparound Plans of Care (WPOC).
• Coordinate transitions between levels of care and adult services when appropriate.
• Maintain documentation in compliance with Spurwink and required regulations.
• Provide weekly face-to-face contacts with families and team members.
