Job summary

Pathways Healthcare is seeking a skilled Home Health Nurse to join our clinician-led team in Kingston, PA. You will initiate preventive and rehabilitative nursing procedures, administer medications, and assess patient needs in the home setting.

The role requires an active RN license, WCC, and BLS CPR, with a preference for 2+ years in geriatrics. A valid driver's license and own vehicle are essential, as is strong communication with physicians and care teams.

Qualifications

• Must hold an active Nursing license in the State(s) where the position is based.
• Must hold an active Wound Care Certification (WCC).
• Preferably 2+ years of nursing experience, with a focus on geriatrics.
• Nursing degree from an accredited institution.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Initiate appropriate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status and evolving needs.
• Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
• Responsively address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them as part of the care plan development.
