Job summary

Mont Marie Rehabilitation and Healthcare Center is seeking a Wound Care Nurse to provide skilled clinical care, wound assessment, and treatments within a sub-acute/long-term care setting in Holyoke, MA. You will support interdisciplinary care planning, maintain HIPAA-compliant documentation, and guide nursing staff.

The role requires MA RN or LPN licensure, BLS CPR, and at least 2 years of wound care experience in long-term care.

Qualifications

• Must hold an active MA RN or LPN license.
• Wound care certification strongly preferred but not required.
• BLS/CPR certification required.
• Minimum 2 years of wound care experience in a nursing home or long-term care setting.

Responsibilities

• Provide resident care in accordance with acceptable nursing standards, physician orders, and established care plans.
• Demonstrate sound clinical judgment in assessing, treating, and monitoring wounds.
• Provide wound care treatments based on resident needs and best clinical practices.
• Deliver respectful, compassionate, and person-centered care to residents, families, and caregivers.
• Provide clinical guidance and direction to members of the nursing and care team.
• Maintain accurate and timely documentation of wound assessments, treatments, and outcomes.
• Ensure resident confidentiality is maintained at all times in accordance with HIPAA and facility policies.
• Collaborate with interdisciplinary teams to support healing, prevention strategies, and continuity of care.

• Must possess a current, unencumbered, active LPN or RN license in the state of MA.
• Wound Care Certification strongly preferred but not required.
• Valid BLS Certification (CPR) required.
• Minimum of 2 years of prior experience as a Wound Care Nurse in a nursing home or long-term care setting required.
• Strong clinical assessment and critical thinking skills.
• Ability to work effectively in a fast-paced healthcare environment.
