Job summary

Mont Marie Rehabilitation invites a Wound Care Nurse to join our team in MA. The role focuses on wound assessment, treatment, and interdisciplinary care planning for residents in a Sub-Acute/Long-Term setting.

Day-to-day duties include accurate documentation and delivering compassionate, person-centered care. The position requires an active MA LPN/RN license, valid BLS CPR, and at least two years of wound care experience in a nursing home or long-term care environment.

Qualifications

• Active MA LPN or RN license required.
• Wound care certification preferred but not required.
• Valid BLS CPR certification required.
• Minimum 2 years wound care experience in long-term care.

Responsibilities

• Provide resident wound care per nursing standards and physician orders.
• Assess, treat, and monitor wounds with sound clinical judgment.
• Document wound assessments and treatment outcomes accurately.
• Collaborate with interdisciplinary teams to support healing.
• Maintain HIPAA-compliant confidentiality at all times.
• Guide and support nursing and care team members.

Responsibilities for Wound Care Nurse:

• Provide resident care in accordance with acceptable nursing standards, physician orders, and established care plans.
• Demonstrate sound clinical judgment in assessing, treating, and monitoring wounds.
• Provide wound care treatments based on resident needs and best clinical practices.
• Deliver respectful, compassionate, and person-centered care to residents, families, and caregivers.
• Provide clinical guidance and direction to members of the nursing and care team.
• Maintain accurate and timely documentation of wound assessments, treatments, and outcomes.
• Ensure resident confidentiality is maintained at all times in accordance with HIPAA and facility policies.
• Collaborate with interdisciplinary teams to support healing, prevention strategies, and continuity of care.

Qualifications for Wound Care Nurse:

• Must possess a current, unencumbered, active LPN or RN license in the state of MA.
• Wound Care Certification strongly preferred but not required.
• Valid BLS Certification (CPR) required.
• Minimum of 2 years of prior experience as a Wound Care Nurse in a nursing home or long-term care setting required.
• Strong clinical assessment and critical thinking skills.
• Ability to work effectively in a fast-paced healthcare environment.
