Parkview Care and Rehabilitation Center in Massapequa, NY seeks an experienced Wound Care Registered Nurse (RN) to join our skilled nursing team. The ideal candidate will manage wound care plans, lead rounds, and document progress for residents.

Requirements include an active NY RN license, Wound Care Certification, and Infection Control Certification, with 1 year LTC experience preferred. This full-time role offers comprehensive benefits and a collaborative work environment.

Qualifications

• Active New York RN license required.
• Wound Care Certification required.
• Infection Control Certification required.
• Minimum 1 year of nursing home or long-term care experience preferred.
• Strong clinical assessment, communication, organizational, and documentation skills.
• Ability to work collaboratively with physicians, nursing staff, residents, and families.

Responsibilities

• Assess residents admitted with wounds and those who develop wounds during their stay.
• Collaborate with physicians and the interdisciplinary team to create wound care plans.
• Lead weekly wound rounds with accurate measurements, assessments, and documentation.
• Maintain weekly wound tracking reports and notify physicians of non-healing wounds.
• Track, analyze, and distribute weekly decubitus ulcer statistics.
• Provide wound care education and training to nursing staff.
• Prepare QA reports to support regulatory compliance and quality improvement.
• Observe dressing changes for proper technique and staff competency.
• Educate residents and families about wound care plans as appropriate.
• Participate in ongoing wound care education and in-services to stay current.
• Maintain accurate clinical documentation per facility policies and regulations.
