Parkview Care and Rehabilitation Center in Massapequa, NY seeks an experienced Wound Care Registered Nurse (RN) to join our skilled nursing team. You will assess wounds, develop treatment plans, and lead wound rounds to improve resident outcomes.

This full-time role requires NY RN license, Wound Care and Infection Control certifications, and collaborative care with physicians and families. Monday–Friday, 6:00 AM–2:00 PM, with competitive salary up to $120,000 per year, based on experience.

Qualifications

• Active New York State RN license.
• Wound Care Certification.

• Minimum 1 year in nursing home or long-term care preferred.
• Strong clinical assessment, communication, organizational, and documentation skills.
• Ability to collaborate with physicians, staff, residents and families.

Responsibilities

• Assess residents with wounds and those who develop wounds during care.
• Collaborate with physicians and the care team to plan wound treatment.
• Lead weekly wound rounds with measurements, assessments and documentation.
• Maintain weekly wound tracking reports and flag non-healing wounds.
• Track and share weekly decubitus ulcer statistics.
• Provide wound care education to nursing staff.
• Prepare QA reports to support regulatory compliance.
• Educate residents and families on wound care plans.
• Maintain accurate clinical documentation per policies.

Parkview Care and Rehabilitation Center is seeking an experienced and compassionate Wound Care Registered Nurse (RN) to join our skilled nursing team. This position is ideal for a clinically strong nurse who is passionate about wound management, quality outcomes, and providing exceptional care to residents.

Key Responsibilities

• Assess residents admitted with wounds and residents who develop wounds during their stay.
• Collaborate with physicians and the interdisciplinary team to develop and implement individualized wound care treatment plans.
• Lead and participate in weekly wound rounds, including accurate wound measurements, assessments, and documentation.
• Complete and maintain weekly wound tracking reports and promptly communicate non-healing wounds or concerns to physicians.
• Track, analyze, and distribute weekly decubitus ulcer statistics.
• Provide in-service education and training to nursing staff regarding wound care best practices and proper treatment techniques.
• Prepare quarterly Quality Assurance (QA) reports to support regulatory compliance and quality improvement initiatives.
• Observe and evaluate dressing changes to ensure proper technique and staff competency.
• Educate residents and families regarding wound care and treatment plans as appropriate.
• Participate in ongoing wound care education and in-services to remain current with clinical standards and best practices.
• Maintain accurate and timely clinical documentation in accordance with facility policies and regulatory requirements.
