Responsibilities

• Assess residents admitted with wounds and those who develop wounds during their stay.
• Collaborate with physicians and the interdisciplinary team to create wound care plans.
• Lead weekly wound rounds with accurate measurements, assessments, and documentation.
• Maintain weekly wound tracking reports and notify physicians of non-healing wounds.
• Track, analyze, and distribute weekly decubitus ulcer statistics.
• Provide wound care education and training to nursing staff.
• Prepare QA reports to support regulatory compliance and quality improvement.
• Observe dressing changes for proper technique and staff competency.
• Educate residents and families about wound care plans as appropriate.
• Participate in ongoing wound care education and in-services to stay current.
• Maintain accurate clinical documentation per facility policies and regulations.

Salary: Up to $120,000 per year, based on experience

Parkview Care and Rehabilitation Center is seeking an experienced and compassionate Wound Care Registered Nurse (RN) to join our skilled nursing team. This position is ideal for a clinically strong nurse who is passionate about wound management, quality outcomes, and providing exceptional care to residents.

Key Responsibilities

• Assess residents admitted with wounds and residents who develop wounds during their stay.
• Collaborate with physicians and the interdisciplinary team to develop and implement individualized wound care treatment plans.
• Lead and participate in weekly wound rounds, including accurate wound measurements, assessments, and documentation.
• Complete and maintain weekly wound tracking reports and promptly communicate non-healing wounds or concerns to physicians.
• Track, analyze, and distribute weekly decubitus ulcer statistics.
• Provide in-service education and training to nursing staff regarding wound care best practices and proper treatment techniques.
• Prepare quarterly Quality Assurance (QA) reports to support regulatory compliance and quality improvement initiatives.
• Observe and evaluate dressing changes to ensure proper technique and staff competency.
• Educate residents and families regarding wound care and treatment plans as appropriate.
• Participate in ongoing wound care education and in-services to remain current with clinical standards and best practices.
• Maintain accurate and timely clinical documentation in accordance with facility policies and regulatory requirements.
