Avalon Health Care Group is seeking a Wound Nurse RN to coordinate wound care and oversee the skin management program at Avalon Care Center Federal Way, WA. The role involves collaborating with the interdisciplinary team to plan and implement care strategies for residents requiring wound care.

The ideal candidate will have an active RN license in Washington, recent geriatrics experience, and strong communication skills to document care plans and educate residents and families.

Qualifications

• Must hold and maintain an active RN license in the practicing state.
• Recent clinical experience, with geriatrics focus.
• Infection prevention knowledge and practices.
• Continue education credits pursued regularly.
• Proven ability to communicate with staff, residents and families.
• Ability to adapt to a flexible work schedule.
• Positive interactions with residents, families, and staff.

Responsibilities

• Collaborate on individualized care plans with residents and care team.
• Set realistic long- and short-term resident goals.
• Initiate nursing interventions with evaluation timeframes.
• Update plans with input from residents and team.
• Provide teaching aligned with the care plan.
• Deliver optimum resident care and comfort with current nursing practices.
• Transcribe orders accurately and consult for clarification when needed.
• Notify physicians of non-adherence and document events.
• Perform only tasks within RN scope and follow policies.
• Ensure accurate medical records documentation and reporting.
