Avalon Care Center Federal Way is hiring a Wound Nurse LPN to join our compassionate team in Federal Way, WA. You will coordinate wound care and oversee the skin management program for residents.

The ideal candidate holds an active LPN/LVN license, has geriatrics experience, and demonstrates strong communication and documentation skills. Full-time weekday schedule with benefits is offered.

Qualifications

• Must hold and maintain a current LPN/LVN license in the practicing state.
• Recent clinical experience or specialty skills in geriatrics.
• Demonstrate infection control knowledge and best practices.
• Continued education credits in clinical subjects and management.
• Proven ability to communicate effectively with residents, families, and staff.
• Flexibility to adapt to changing schedules and needs.

Responsibilities

• Coordinate the wound care plan with the resident and care team.
• Set realistic short- and long-term goals for residents.
• Initiate nursing interventions with timeframes and document outcomes.
• Revise care plans with input from residents and team.
• Provide teaching consistent with the interdisciplinary plan.
• Deliver optimum resident care and comfort using current nursing practices.
• Transcribe orders accurately and clarify as needed.
• Notify physicians if orders are not carried out and document.
• Perform activities within LPN/LVN scope of practice.
• Follow policies and use nursing judgment appropriately.
• Ensure accurate medical record documentation and reporting.
