Job summary

The position requires a Diploma in Nursing and an Indiana RN license, with BLS or instructor certification within 30 days of hire. Wound care experience is preferred and 2+ years of RN experience is advantageous.

Responsibilities

• Evaluates patient discharge planning needs in collaboration with the healthcare team.
• Establishes, tracks, and manages patient care plans while observing, documenting, and reporting status updates.
• Acts as the main coordinator across all medical disciplines to deliver seamless care.
• Fulfills and reviews nursing and physician directives.
