Qualifications

Responsibilities

• Assess and monitor wounds. Clean and bandage as required.
• Implement the wound/ostomy/continence treatment plan of care.
• Explain procedures, medications, and treatment to patient and authorized friend or family member.
• Assist medical providers with treatments and procedures as necessary.
• Monitor patient for adverse reactions to treatments.
• Administer medications as applicable, knowing their actions and side effects.
• Document patient findings, condition, visit details, plan of care, discharge plan and other pertinent input.
• Delegate tasks to other team members as needed.
• Contribute to discharge planning for continuity of care post hospitalization.
• Other duties as assigned.
