Kensington Gardens Rehab is seeking a Wound Care Nurse to develop and implement wound care plans, monitor healing progress, and educate patients and families. The role emphasizes comprehensive skin-care management, weekly wound reviews with a consultant, and coordination of follow-up care.

The position requires BSN or equivalent, active RN/LPN license, and CPR/BLS certification, with two years of nursing experience and preferred geriatric/subacute experience. Equal opportunity employer.

Qualifications

• BSN or equivalent required; RN or LPN licensure in the state of employment.
• Current RN/LPN license in the state employed is required.
• Peripheral IV certification is required.
• CPR/BLS certified.
• Minimum two years of clinical nursing experience.
• One year of management experience preferred.
• One year in geriatrics/subacute care preferred.

Responsibilities

• Maintain a comprehensive care plan for skin impairments and update as needed.
• Conduct weekly wound rounds with wound consultant.
• Document weekly wound measurements.
• Review investigations for new skin openings related to pressure.
• Monitor vascular/arterial/surgical wounds and coordinate follow-up.
• Educate and counsel patients and families on wound care processes.
• Demonstrate wound care procedures to other caregivers.
