Hillside Health and Rehabilitation is seeking an experienced Wound Care Nurse (RN) to provide specialized wound management for residents. You will assess wounds, develop treatment plans, monitor healing, and collaborate with the interdisciplinary team to ensure safe, effective outcomes.

This role requires an active RN license, CPR/BLS certification, and at least two years of clinical nursing experience, with geriatrics or subacute care preferred.

Formación

• BSN preferred; accredited RN program graduate required.
• Current, active RN license.
• Peripheral IV certification.
• Minimum of 2 years of clinical nursing experience.
• Geriatrics or subacute care experience preferred.
• Management or leadership experience a plus.
• CPR/BLS certification.

Responsabilidades

• Maintain accurate and updated care plans for all skin impairments.
• Conduct weekly wound rounds with the wound consultant.
• Perform and document weekly wound measurements with accuracy and timeliness.
• Monitor and evaluate all new pressure-related skin openings and determine root causes.
• Assess and manage surgical, arterial, venous, and other complex wounds.
• Ensure all treatments, changes, and follow-up needs are documented thoroughly.
• Educate residents, families, and caregivers on wound care procedures and expectations.
• Demonstrate wound care techniques to caregivers to support staff competency.
• Communicate with the interdisciplinary team to support coordinated care.
