Job summary

Kensington Gardens Rehab seeks a Wound Care Nurse to provide specialized wound care, create treatment plans, monitor infections, and coordinate care for patients with wounds from ulcers, surgical sites, and other skin impairments. The role includes patient education and collaboration with the wound care team.

Qualifications include a BSN or RN/LPN license, peripheral IV certification, CPR/BLS, and at least two years of clinical nursing experience, with preference for geriatrics and management

Qualifications

• BSN or equivalent required.
• Graduate of RN or LPN nursing program.
• Current RN/LPN license to practice in the state employed.
• Peripheral IV certification required.
• Two years of clinical nursing experience required.
• One year of management experience preferred.
• One year of geriatrics and subacute experience preferred.
• CPR/BLS certification required.

Responsibilities

• Maintain a Comprehensive care plan for skin impairments and update as needed.
• Conduct weekly wound rounds with wound consultant.
• Document weekly wound measurements.
• Review investigations for new skin openings related to pressure.
• Monitor vascular/arterial/surgical wounds and document treatment and follow-up.
• Educate patients and families on wound care processes and issues.
• Demonstrate wound care procedures to other caregivers.

Wound Care Nurse

The Wound Care Nurse provides care that will quickly and safely heal various types of wounds, including surgical openings, ulcers, bedsores, feeding tube sites, and abscesses. The wound care nurse will access the wounds, create and implement treatment plans, monitor infection and signs of various infections, and clean wounds.

RESPONSIBILITIES:

• The Wound Care Nurse maintains a Comprehensive care plan for skin impairments- updating as needed to include any change in treatment, improvement and/or decline
• Weekly wound rounds with wound consultant
• Weekly wound measurements documented
• Review investigation for all new skin opening related to pressure
• Monitoring vascular/arterial/surgical wounds and document appropriately -treatment, comprehensive care plan, coordinating follow-up when indicated & maintain weekly measurements
• Educate and counsel patients and their families on wound care processes and issues
• Demonstrate wound care procedures to other care givers

QUALIFICATIONS:

• BSN or equivalent
• Must be a graduate of an RN or LPN nursing program
• Current registration and license to practice as a Registered Nurse OR Licensed Practical Nurse in the state employed in
• Peripheral IV certification
• Two years of clinical nursing experience
• One-year experience in a management position preferred
• One-year experience in geriatrics and subacute population
• CPR/BLS certified
