Job summary

St. James Manor & Villas is seeking an RN Wound Care Coordinator to oversee wound care management and skin integrity programs for residents. The role emphasizes high‑quality wound care, preventative strategies, and compliance with all regulations and facility policies.

You will lead wound rounds, document conditions, and collaborate with physicians and interdisciplinary teams to support resident healing and overall well‑being.

Qualifications

• Current RN license required by state.
• CPR certification required.
• Minimum two (2) years of RN or LPN experience.
• Ability to lift up to 75 lbs.
• Strong English communication skills.

Responsibilities

• Apply the nursing process for wound and skin care assessment and planning.
• Conduct regular wound care rounds with timely documentation.
• Assess resident skin integrity and document related medical history.
• Implement preventative wound care strategies in care plans.
• Prioritize and execute nursing interventions for wound management.
• Coordinate with physicians and interdisciplinary teams on wound treatment needs.
• Initiate nutritional interventions to support wound healing.
• Deliver direct resident care per facility policies.
• Ensure proper pericare and preventative measures for incontinence cases.
• Maintain safe, clean work environment following infection control standards.

We are seeking a dedicated RN Wound Care Coordinator to oversee wound care management and skin integrity programs for our residents. The Wound Care Nurse is responsible for providing high-quality wound care, ensuring preventative measures are in place, and maintaining compliance with federal and state regulations, as well as facility policies.

RN Wound Care Coordinator Requirements:

• Active Nursing License (RN) as required by the state
• Current CPR certification
• Minimum two (2) years of experience as an RN or LPN
• Ability to lift and move patients and items up to 75 lbs
• Strong vision abilities including close, distance, peripheral, and depth perception
• Excellent verbal and written communication skills in English
• Ability to cope with the mental and emotional stress of the position
• Strong attention to detail and ability to prioritize wound care needs

As a Skilled, Compassionate RN Wound Care Coordinator, You Will:

• Utilize the nursing process for assessment, planning, implementation, and evaluation of wound and skin care
• Conduct regular wound care rounds, ensuring timely documentation and tracking of resident skin conditions
• Assess resident skin integrity and document medical history related to wound care
• Implement preventative wound care strategies in resident care plans
• Prioritize and execute nursing interventions for wound management
• Communicate with physicians and interdisciplinary teams regarding wound treatment needs
• Initiate nutritional interventions to support wound healing
• Perform direct resident care in compliance with facility policies and procedures
• Ensure residents with incontinence receive appropriate pericare and preventative treatments
• Maintain a safe and clean work environment, adhering to infection control protocols
