• Assess a comprehensive evaluation of physical, social, and psychological status and coordinate with the interdisciplinary team.
• Develop individualized care plans with patient and family involvement and plan for ongoing education.
• Direct and coordinate the care team to deliver care per plan, manage time and resources.
• Evaluate patient responses and revise the care plan as needed.
• Maintain accurate documentation and uphold professional and legal standards.
