Summary of Primary Function/General Purpose of Position

The Unboarded Graduate has graduated from an accredited nursing program but has not passed the National Council Licensure Examination (NCLEX). Functioning as a member of the Care Delivery Team, the Unboarded RN Graduate works under the direction of a Clinical Nurse to provide direct, quality nursing care for patients and performs duties as assigned. The Unboarded RN Graduate must progress to the Registered Nurse (RN) New to Practice as applicable in accordance with the State Board of Nursing licensing requirements within 90 days of hire. If licensure requirement is not met due to not passing NCLEX, license must be obtained within 60 days of NCLEX fail date.

Essential Job Functions
  1. Performs nursing/medical treatments and diagnostic tests and procedures according to policy/procedure with guidance and supervision from a Registered Nurse which may include, but not limited to:
  • Takes and records vital signs, pulse oximetry, and weights.
  • Collects, labels, and sends specimens to lab.
  • Observes, measures, and records intake of P.O. fluids and tube feedings and output of urine, drainage, vomitus, stools, suction canisters, and chest tubes.
  • If trained and deemed competent, inserts and discontinues intermittent and continuous urinary catheters and performs peripheral phlebotomy.
  • Performs Point of Care Blood Glucose testing, oral suctioning, chest PT, turn/cough/deep breathing, sterile and non-sterile dressing changes and wound care, and 12 lead ECGs.
  • Applies elastic bandages, splints, and anti-embolitic stockings.
  • Independently transports stable patients to testing areas.
  • Observes patients and keeps responsible licensed staff informed of general condition and/or change in their status. Appropriately documents treatments, data, and care provided in the patient’s electronic health record.
  • Assists patients’ personal hygiene activities such as bathing, dressing, hair care, oral hygiene, back and skin care, perineal care, and indwelling urinary care.
  • Feeds patients on oral and tube feedings.
  • Prepares, stocks, and maintains patient room/bedside with appropriate equipment and supplies for patient admission, transfer, and/or discharge.
  • Sets up tube feedings, pressure, and IV lines under guidance and supervision of Clinical Nurse/preceptor.
  • Sets up and assists in maintaining equipment including but not limited to oxygen, suction, bedside monitors, transport equipment.
  • Identifies issues related to supplies, equipment, and environmental safety and communicates findings following the department’s chain of command.
  • Collaborates with supervisor to identify own learning needs and set goals and uses available resources to meet these needs/goals.
  • Participates in clinical in-services and utilizes information to enhance patient care activities and work behaviors; participates in unit-based continuous improvement activities.

Patient Population

Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. The candidate should demonstrate understanding of growth and development principles across the lifespan and the ability to assess data reflective of the patient's status to provide appropriate care, as per departmental policies. The patient populations include:

  • Neonates (0-4 weeks)
  • Infants (1-12 months)
  • Pediatrics (1-12 years)
  • Adolescents (13-17 years)
  • Adults (18-64 years)
  • Geriatrics (65 years and older)
