Job summary

Schervier Rehab and Nursing Center in the Bronx is seeking a dedicated Registered Nurse (RN) to join our team as a Wound Care Nurse. You will perform wound assessments, develop individualized treatment plans, and document treatments according to physician orders and facility protocols.

You will monitor healing progress, update physicians and families, and collaborate with the interdisciplinary care team to ensure continuity of care.

Qualifications

• Must be a licensed Registered Nurse (RN) in New York State.
• Prior wound care experience required (WCC certification preferred).
• Strong assessment, documentation, and communication skills.
• Compassionate and professional demeanor with a commitment to quality care.

Responsibilities

• Perform comprehensive wound assessments and develop individualized treatment plans
• Provide and document wound care treatments per physician orders and facility protocols
• Monitor wound healing progress and communicate updates to physicians and families
• Collaborate with the interdisciplinary care team to ensure continuity of care
• Educate nursing staff on wound prevention, care techniques, and infection control practices
• Maintain compliance with all regulatory standards and facility policies
