Job summary

Yale New Haven Health is seeking a Nurse Practitioner or Physician Assistant to join the Wound Care & Ostomy Services team at Westerly Hospital, with a Monday–Friday schedule and no weekend calls. This full-time role supports inpatient, outpatient, and ambulatory care across two sites, using EPIC for documentation.

Responsibilities include comprehensive wound care assessment and management, patient education, and coordination with the interdisciplinary team.

Qualifications

• Minimum of one year of wound care experience.
• Board certification for NP or PA and active licensure in RI and CT.

Responsibilities

• Provide assessment, triage, and therapeutic management of wound and ostomy patients as part of an interdisciplinary team.
• Educate patients and families on care plans and resources.
• Document care and ensure compliance with regulatory requirements.
• Participate in quality management and ongoing professional development.
• Collaborate with supervising physicians on treatment plans and pathways.
